I practice in the Home-Based Medicine department and the Advanced Care Primary Care Clinic, caring for older adults living with multiple chronic conditions, functional limitations or serious illness. My clinical background in internal medicine and palliative care allows me to actively manage complex medical conditions while maintaining a strong focus on quality of life and symptom control. I frequently care for patients with congestive heart failure, chronic lung disease such as COPD, dementia, chronic kidney disease and type 2 diabetes. My approach centers on understanding each patient’s priorities, developing proactive care plans aligned with their goals, and coordinating care with specialists and therapists when needed. Some patients I care for may live for many years, while others face more limited time. Regardless, my goal remains the same: to understand what matters most to each person and help them live as well as possible for as long as they can.
