Dr. Shoshana Streiter joined the division in 2019. She is board certified in geriatrics and internal medicine. She leads the HOPE (Hospitalized Older Person Experience) pathway, a co-management service between Geriatrics and Internal Medicine, which she initiated in 2019.The HOPE pathway serves hospitalized older adults at high risk for geriatric syndromes such as delirium, cognitive impairment, falls, or polypharmacy. HOPE provides an extra layer of support for vulnerable older adults in the inpatient setting and offers education about managing older adults in an organic and integrated way to the Internal Medicine trainees. She is committed to the improvement of care received by older patients and to further developing co-management models of care for these patients.Dr. Streiter is also a dedicated educator. As a geriatrics fellow at BIDMC she was on the Clinician Educator Track and during her advanced Geriatric fellowship at the VA Boston Healthcare System (VABHS) she focused on geriatric medical education, with a concentration on curriculum design and implementation. She participated in the 2018 Harvard Macy Institute Program for Post-Graduate Trainees: Future Academic Clinician-Educators and is currently participating in the 2021-22 Harvard Macy Institute Program for Educators in Health Professions.
