What is the definition of Chronic Antibody-Mediated Rejection?
Chronic antibody-mediated rejection (AMR) is a human leukocyte antigen (HLA) antibody-induced major complication that occurs after solid organ (kidney, lung, pancreas, and intestinal) transplantation that causes ongoing graft injury and graft loss. Chronic antibody-mediated rejection usually occurs more than one year after transplant.
What are the symptoms of Chronic Antibody-Mediated Rejection?
Symptoms of chronic antibody-mediated rejection can depend on the type of organ transplanted; however, the condition is characterized by endothelial cell changes, microvascular inflammation, C4d (complement protein splitting), circulating donor-specific antibodies, fibrosis, graft dysfunction, and probable graft loss. Patient symptoms may also include dyspnea (difficult breathing), cough, fever, malaise, or they may be asymptomatic.

What are the current treatments for Chronic Antibody-Mediated Rejection?
Currently, there are few therapies that can prevent and reverse chronic antibody-mediated rejection. However, patients are often treated with a range of therapies, such as plasma exchange (PLEX); intravenous immunoglobulin (IVIG); complement inhibitors, such as eculizumab and rituximab; pulse steroids; imlifidase; anti-thymocyte globulin (ATG); or other T-cell-depleting antibodies; the proteasome inhibitor, bortezomib; cyclophosphamide; interleukin-6 inhibitors, and a triple immunosuppressants protocol, which includes mycophenolic acid, tacrolimus, and steroids.
