Linzess
What is Linzess (Linaclotide)?
Linzess (linaclotide) is a prescription medication specifically designed to treat chronic gastrointestinal conditions that affect millions of adults. The FDA has approved Linzess for two primary conditions: Chronic Idiopathic Constipation (CIC) and Irritable Bowel Syndrome with Constipation (IBS-C) in adults aged 18 and older.
The medication works by activating guanylate cyclase-C receptors in the intestines, which increases both intestinal fluid secretion and gastrointestinal transit. This mechanism helps ease constipation and reduce abdominal pain associated with these chronic conditions. Unlike traditional laxatives, Linzess addresses both the motility issues and pain sensations that characterize IBS-C, making it a unique therapeutic option.
For patients with IBS-C, Linzess helps alleviate the constellation of symptoms that typically include:
- Chronic constipation
- Abdominal pain and discomfort
- Bloating
- Difficult or incomplete bowel movements
- Hard or lumpy stools
What are the side effects of Linzess?
As with any medication, Linzess can cause various side effects ranging from mild to more severe. The most common side effect, experienced by many patients, is diarrhea. This typically occurs within the first two weeks of treatment, though it may appear at any time during treatment.
Common side effects that patients may experience include:
- Diarrhea (most common)
- Gas
- Abdominal pain
- Bloating
- Flatulence
- Stomach flu-like symptoms
More serious side effects, though rare, may include:
- Severe diarrhea leading to dehydration
- Severe abdominal pain
- Fainting or dizziness due to dehydration
- Allergic reactions
It’s particularly important to note that Linzess carries a black box warning (the FDA’s strongest warning) contraindicating its use in patients aged 6 years and younger, and warning against use in children aged 6 to 17 years. This is due to safety concerns observed in young animal studies.
To minimize the risk of side effects, healthcare providers typically recommend: • Taking Linzess on an empty stomach • Taking the medication at least 30 minutes before the first meal of the day • Staying well-hydrated throughout the day • Contacting a healthcare provider if severe diarrhea occurs
Is there a generic drug version of Linzess?
As of 2023, there is no FDA-approved generic version of Linzess available in the United States. The medication remains under patent protection, which prevents generic manufacturers from producing equivalent versions. The earliest possible date for generic availability is anticipated to be in the latter part of this decade, though exact timing depends on various factors including patent extensions and legal challenges.
This lack of generic alternatives means that Linzess can be relatively expensive for patients, particularly those without adequate insurance coverage. However, the manufacturer offers various patient assistance programs and savings cards to help make the medication more affordable for eligible patients. These programs can significantly reduce out-of-pocket costs for those who qualify.
Linzess Dosage Information
Linzess comes in capsule form and is available in three different strengths: 72 mcg, 145 mcg, and 290 mcg. The appropriate dosage depends on the condition being treated and individual patient response to the medication.
For Chronic Idiopathic Constipation (CIC):
- The recommended starting dose is 72 mcg once daily
- If needed, the dose can be increased to 145 mcg once daily
- Some patients may require the maximum dose of 290 mcg daily
For Irritable Bowel Syndrome with Constipation (IBS-C):
- The recommended dose is 290 mcg once daily
- This higher dose is needed to address both the constipation and pain components of IBS-C
Important dosing considerations include:
Administration Guidelines:
- Take Linzess once daily on an empty stomach
- Take at least 30 minutes before the first meal of the day
- Swallow the capsule whole – do not crush, chew, or open
- If a dose is missed, skip it and take the next dose at the regular time
- Do not take more than one capsule per day
Special Populations:
- No dose adjustments are necessary for elderly patients
- No dose adjustments are needed for patients with kidney or liver impairment
- Not recommended for use in patients under 18 years of age
Healthcare providers will typically start with the lowest effective dose and adjust based on individual response and tolerability. It’s important to note that Linzess may take several days to several weeks to show full effectiveness, and patients should not adjust their dosage without consulting their healthcare provider.
The medication should be stored at room temperature, away from moisture and heat. Like all medications, Linzess should be kept out of reach of children and pets. Regular follow-up with healthcare providers is important to monitor the effectiveness of treatment and address any side effects or concerns that may arise during therapy.

Summary: The purpose of this study is to determine if adult patients with IBS-C will report an overall greater improvement in IBS symptoms and quality of life when treated with a combination of linaclotide (standard of care medication) and immersive virtual reality (VR) therapy compared to those treated with linaclotide and sham (placebo) VR therapy.
