Jacob Van Fleet, MD, is a board-certified family medicine physician who partners with his patients to provide them with high-quality, broad-spectrum care. He strives to strengthen the health of the community by caring for those in need. From chronic disease to basic office procedures, from pediatrics to geriatrics, he hopes to build partnerships with individuals in all facets of life.
