He completed his ophthalmology residency at New York Medical College, serving as Chief Resident at Westchester Medical Center, followed by fellowship training in neuro-ophthalmology at Columbia Presbyterian Medical Center and Manhattan Eye, Ear & Throat Hospital under Drs. Myles Behrens and Jeffrey Odel. In addition to his clinical and academic expertise, Dr. Forman incorporates functional medicine and nutritional biochemistry into patient care, offering a holistic approach to vision and overall health. He currently sees patients at Belchertown Eye Care and holds hospital privileges at Cooley Dickinson Hospital. Outside of medicine, he enjoys playing jazz and blues guitar, swimming, and bicycling.
