      I study disorders of the bone marrow and bone marrow failure syndromes. I also see some leukemias and lymphomas and other blood malignancies. Duke offers a very personalized, individualized level of care to each patient. Nobody is the same, so we take the whole patient, we listen to them, and we offer our best recommendations for treatment options.       Since studying basic immunology prior to beginning my clinical training, I have been deeply interested in applying immunological perspective for the treatment of cancers including hematologic malignancies.       What makes Duke Health unique in bone marrow transplants is its holistic approach. We take care of the disease -- which is important, because these are very serious, potentially fatal diseases.       The Division of Cellular Therapy does bone marrow transplants and stem cell transplants for patients with different types of blood cancers. Specifically, I see patients with multiple myeloma or other plasma cell issues like amyloidosis. Many of the cases in my clinic are difficult.       What is the definition of Multicentric Castleman Disease (MCD)?

      Multicentric Castleman Disease (MCD) is a rare disease that affects the lymph nodes and related tissues. It is a form of Castleman disease that is "systemic" and involves multiple regions of lymph nodes (as opposed to unicentric Castleman disease, which involves a single lymph node or single region of lymph nodes). The signs and symptoms of MCD are often nonspecific, and are mild in some people but serious in others. Symptoms may include enlarged lymph nodes in multiple regions, fever, weight loss, nausea, rash, and/or an enlarged large liver and spleen. The disease is diagnosed based on the symptoms present, laboratory test results, imaging studies, and results of a biopsy of the lymph nodes which shows specific features when studied under the microscope. In some cases, MCD is caused by human herpesvirus-8 (HHV-8) and is referred to as HHV-8-associated MCD. In other cases, the cause is not known and it is referred to as HHV-8 negative MCD, or idiopathic MCD (iMCD).
