      My clinical and research interests focus on disorders caused by telomere dysfunction. Our goal is to define approaches to surveillance, diagnosis and treatment for patients with telomere-mediated disease. Human syndromes associated with telomere shortening were first identified in the context of dyskeratosis congenita, a premature aging syndrome that predisposes to cancer. Dyskeratosis congenita patients and patients with telomere-mediated syndromes have an increased risk for developing certain malignancies which include myelodysplastic syndromes, myeloid leukemia and squamous cell carcinomas of the upper aerodigestive tract. The Telomere Clinic at the Sidney Kimmel Comprehensive Cancer Center at Johns Hopkins provides multi-disciplinary care to patients who are suspected to have or who carry the diagnosis of telomere-related disorders. These disorders include individuals with cancer, bone marrow failure/aplastic anemia, lung disease including idiopathic pulmonary fibrosis and liver cirrhosis, among others. Our clinic brings together geneticists, genetic counselors, and we work closely with expert physicians in adult and pediatric hematology, bone marrow transplant, pulmonary medicine, lung transplant medicine, hepatology, and otolaryngology. Together, our team has established expertise in the management of these disorders and is at the forefront of leading the effort to individualize care for affected patients and their families. My group also has ongoing research efforts which include a long-term study to understand the genetics and spectrum of telomere disorders through a Registry which has been based at Johns Hopkins since 2005.       Roy Brower is a Professor of Medicine at Johns Hopkins University and was Medical Director of the Medical Intensive Care Unit at Johns Hopkins Hospital for over 30 years. He obtained his M.D. degree and completed his residency and fellowship in Pulmonary and Critical Care Medicine at Johns Hopkins. Brower’s earlier research explored mechanisms of cardiopulmonary interactions and pulmonary circulation physiology. He subsequently focused research efforts in the clinical venue. Brower has led four multicenter clinical trials of lung-protective mechanical ventilation strategies in patients with the acute respiratory distress syndrome (ARDS). One of these trials demonstrated substantially improved clinical outcomes in ARDS patients who received mechanical ventilation with lower tidal volumes. Brower was a member of the NIH ARDS Network from its inception in 1994. He was the Chair of the Steering Committee of the NIH Prevention and Early Treatment of Acute Lung Injury (PETAL) Network from 2014 to 2023. He served on the Program Committee of the Pulmonary Circulation Assembly and chaired the ATS Critical Care Assembly Planning Committee for two years. He served as chair of the Critical Care Assembly from 2003-2005 and is the recipient of the Critical Care Lifetime Achievement Award. Brower chaired the ATS Publications Policy Committee from 2012 to 2016 and twice received the ATS Presidential Citation. Brower is an accomplished clinical educator.       Errol L. Bush, M.D., FACS, is a thoracic surgeon and surgical director of the Advanced Lung Disease and Lung Transplant Program for the Johns Hopkins Comprehensive Transplant Center.
