Hello, I’m Sommer Perkins, PMHNP-BC, a board-certified Psychiatric Mental Health Nurse Practitioner licensed in Colorado, New Mexico, and Montana.
I founded Out West Mental Health to provide accessible, compassionate, and individualized psychiatric care through convenient, private telehealth. I work with adults experiencing ADHD, anxiety, depression, OCD, panic, sleep concerns, and other mental health challenges.
My approach is collaborative and grounded in conservative, evidence-based prescribing. I believe effective care begins with understanding the whole person—not simply treating a diagnosis. I partner with each client to develop a treatment plan that reflects their goals, needs, and preferences.
My goal is to create a space where you feel heard, respected, supported, and never judged.
