Dr. Prachi T. Parikh
Overview
I am a neurologist and specialize in epilepsy. I see patients who have seizures and help them decide on a comprehensive plan for their diagnosis and treatment. I treat patients with seizures that are difficult to control. I also treat patients that have failed several medications and are looking for other options, such as surgery, clinical trials, and neuromodulation devices, such as responsive neurostimulation (RNS), vagal nerve stimulation (VNS), and deep brain stimulation (DBS). I also have an interest in treating female patients with epilepsy and the role hormones can play in their seizures (eg. pregnancy). I focus not only on helping my patients control their seizures, but also addressing the other issues that are associated with being an epilepsy patient, such as depression, quality of life, medication side effects, other neurological issues, and social issues. At Duke Health, we have conferences where our entire team of epileptologists, neurologists, neurosurgeons, technicians, and neuropsychologists meet to discuss individual patients in order to offer the appropriate management. We believe in the timely identification of patients with seizures that are difficult to control and interventions to improve their quality of life.

With over 15 years of experience in the field of electroencephalography, epilepsy diagnosis, and the management of people with drug-resistant epilepsy, I serve as the Director of the Duke Comprehensive Epilepsy Center. My responsibilities also encompass overseeing the Analytical Neurophysiology Lab, which focuses on advancing the field of neurophysiology to more accurately localize epileptic foci in patients with drug-resistant epilepsy, employing state-of-the art signal analysis techniques integrated with artificial intelligence. The ultimate aim of this endeavor is to optimize treatment efficacy through individually tailored approaches, thereby enhancing the quality of life in people with epilepsy. Central to my approach is the concerted effort to assemble a highly collaborative team to achieve the best possible outcome for each patient. To do so, I also strive to integrate the latest innovative developments in the field into patient care, ensuring the highest treatment standards in our field. I am currently the chief of the division of Epilepsy, Sleep, and Clinical Neurophysiology. I see epilepsy patients and patients with sleep disorders. The epilepsy patients that I see often have a hard time controlling their seizures or have other epilepsy-related problems. At Duke, we have many options, including clinical trials, advanced medications, and surgical options, that we can offer patients to get them to a better quality of life with fewer or no seizures. In sleep disorders, I see many patients with sleep apnea and have a passion for treating individuals with narcolepsy. My philosophy is that I want patients to be aware of all the different treatment options that they have available this them. Together, we come up with what I, the patient, and the family think is the best approach for the individual. This way, we can individualize the treatment for each person. It is very gratifying for me to see patients doing so much better with the appropriate medications and pursuing the dreams they have and doing the things they really want to do.
