Multidisciplinary care focus on stroke, spinal cord, neuromuscular, amputee, trauma, medically complex, and brain injury patient populations with a focus on maximizing functional outcomes, support with disability through the course of months to years out. Assist in DME, therapy, psychosocial, and community resources. Medication management for disability including spasticity, cognitive, and emotional needs.
